In the short story essay Greasy Lake by T. Coraghessan Boyle, a literary devise applied is setting. The three different types of setting are physical, historical and geographic. He employs them threw out the essay giving us detailed information on what is going on. Boyle describes the lake as to be a place where some rather not go anymore do to its physical condition. The lake is physically described as â€Å"fetid and murky, the mud banks glittering with broken glass and strewn with beer cans and the charred remains of bonfires† pg 77. The narrator continues to describe the lake with such a horrid atmosphere when he and his friends arrive. Describing all things that come about in a place like that â€Å"the bad breath of decay all around me† pg 80, signifying that the Greasy Lake was not a place to be. In the Greasy Lake, the lake was once described by the narrator as a place that at one time would be described as Wakan. Wakan is the term the narrator tells us the Indians had used to describe the lake. The narrator tells us that the Indians used the term to describe â€Å"the clarity of its waters† pg 77. The now noticeable change of the water from what the Indians considered as â€Å"Wakan† to being â€Å"fetid and murky† shows the significant change in the way society had felt to keep the lake. The dramatic turn is implied when the lake is described to be a place of where I stated above that of â€Å"breath of decay.† Also showing a change in society the narrator focuses on the use of drugs, violence, alcohol, and rape as to be reasons why the lake is the way it is.

Ethical and Legal Challenges in Professional Practice Essay

The American Counseling Association (ACA) Code of Ethics is available to clarify the ethical responsibilities for professional counselors and future professional counselors. According to the ACA (2005), â€Å"the code serves as an ethical guide designed to assist members in constructing a professional course of action that best serves those utilizing counseling services and best promotes the values of the counseling profession.† As a graduate student striving to achieve a Master’s Degree in Counseling, it is crucial, not only to know and understand the ACA Code of Ethics, but also to understand any challenges that I may have in upholding them as well as ways to address these challenges effectively. In this paper I examine a section of the ACA Code of Ethics that I find personally challenging, risk management strategies used to resolve this potential ethical conflict, and a section of the ACA Code of Ethics that will not present a challenge. Personally Challenging Ethics Code According to the ACA Code of Ethics (2005), section C.2.g Impairment, counselors are alert to the signs of impairment from their own physical, mental, or emotional problems and refrain from offering or providing professional services when such impairment is likely to harm a client or others. The ACA Code of Ethics (2005) section C.2.g Impairment also states that counselors seek assistance for problems that reach the level of professional impairment, and, if necessary, they limit, suspend, or terminate their professional responsibilities until such time it is determined that they may safely resume their work. Personal Relevant History In 2005, during my senior year of undergraduate school at The University of  Arizona, I was on the fast track to law school. I was on a full scholarship, earning a 4.0 grade point average, a resident assistant for the dorms, and a member of a co-ed pre-law fraternity. I had just completed my internship working for senator John McCain and had finished the scary LSAT. This is what my friends and family saw. In the background, I was struggling. During the weekends I was â€Å"hosting† parties, or rather people would just show up and throw their own parties at my residence. I was struggling to get out of bed in the morning and often came back home to take naps and miss my next few classes of the day. My grades were slipping and so was my ambition. I took it upon myself to see a psychiatrist and was prescribed anti-depressants. This medication changed my life for the worst. I did not even notice that things were spinning out of control as I maxed out my credit cards (I would just get new cards later) and making impulsive and risky decisions. I was losing sleep as I was either out socializing or home cleaning like a madwoman, and often had bouts of irritability. My boyfriend at the time (my current husband) called my parents and asked that I come home to Phoenix and receive help. So I had a medical withdrawal from school, returned home, and was provided with psychiatric help. I was diagnosed with bipolar disorder and informed that by taking antidepressants I was experiencing a manic episode. As stated by Griswold and Pessar (2000, p. 1347) while referring to bipolar disorder, â€Å"the use of tricyclic antidepressants should be avoided because of the possibility of inducing rapid cycling of symptoms.† So with a new diagnosis the process of trial and error with psychotropic and mood stabilizing medications and their unavoidable side effects began. Once I was on a stable medication and dosage, I felt like myself again. I got a job at a residential treatment center to work with adolescents that have mood disorders and had gotten into trouble with the law. I found my passion. It was a few years before I could return to school with a purpose. I was graduated from Arizona State University with a Bachelor’s of Science degree in Family Studies and Human Development in May 2011 and the future goal of becoming a therapist. Future Considerations and Risk Management  Strategies I believe that under the Impairment ethics code, mood disorders are considered a mental or emotional problem that may impair the counselor affecting the way in which a counselor provides treatment to clients. Bipolar disorder does not disappear once one takes the necessary medication. â€Å"Medication alone is often inadequate to restore and maintain physical health and quality of life† (Rheineck & Steinkuller, 2009, p. 339). Rheineck and Steinkuller (2009) recommend that those with bipolar disorder participate in therapy in conjunction with taking their effective medication. It would be myopic of me to assume that bipolar disorder will never affect me as a therapist. If I am not aware of my moods while I am having either a depressive or manic episode I may become irritable with or place my own perceptions onto a client. Ethically, to manage the risks involved with being a therapist who has bipolar disorder, I need to do more than take medication and participate in therapy. According to Biegel, Brown, & Shapiro (2007), a therapist should practice self-care, including self-awareness and self-regulation or coping. I think that when I am practicing, it will be self-awareness that will assist me most in terms of risk management. As an unbiased observation of my inner experience and behavior, self-awareness could also serve as an alarm to signal that I need to take appropriate actions whether to notify my supervisor, limit, or suspend my professional responsibilities. When referring to self-awareness Corey, Corey, & Callanan (2008, p. 44) state that without it â€Å"mental health professionals are likely to obstruct the progress of their clients as the focus of therapy shifts from meeting the client’s needs to meeting the needs of the therapist.† To assist with my self-awareness, I plan to utilize mindfulness. Mindfulness, as defined by Campbell and Christopher (2012, p. 215), â€Å"refers to a state of being aware, with acceptance, of thought s, emotions, and sensations as they arise.† I currently practice various mindfulness exercises in therapy to assist with my mood disorder and coping strategies. To be a positive and healthy professional counselor I will continue with mindfulness exercises throughout my career and my life. I plan on practicing this daily, on my own time, so that I will be able to recognize when I am having moods or episodes that need to be addressed.  Mindfulness will be additionally useful, as â€Å"counselors need to be immediately cognizant of signs of stress and burnout and address these immediately to practice counseling ethically† (Bradley, Brogan, Brogan, & Hendricks, 2009, p. 358). By being mindful and self-aware I will be able to identify the symptoms of stress and burnout as well as any number of potentially harmful feelings. Ethics Code that Does Not Present a Challenge According to the ACA Code of Ethics (2005), section C.2.f Continuing Education, counselors recognize the need for continuing education to acquire and maintain a reasonable level of awareness of current scientific and professional information in their fields of activity. The ACA Code of Ethics (2005) section C.2.f Continuing Education also states that counselors take steps to maintain competence in the skills they use, are open to new procedures, and keep current with the diverse populations and specific populations with whom they work. Personal Relevant History In my experience while working in behavioral health, training always has been emphasized and mandated yearly. During the four years that I spent working at a residential treatment center, I had accumulated more than 500 hours of training. While working at a group home for a year, I had gone through more than100 hours of training. In the past year while working as a youth and family specialist I have completed an additional 60 hours of training. Although I found many of the training sessions over the years to be fairly repetitive, there were also several trainings providing completely new knowledge to me and therefore effective to assisting me while working with clients. Examples of recent effective trainings include crisis prevention intervention, compassion fatigue, cognitive behavioral therapy for children and adults, and behavioral health documentation. Future Considerations According to the ACA Code of Ethics (2005), Preamble, â€Å"inherently held values that guide our behaviors or exceed prescribed behaviors are deeply ingrained in the counselor and developed out of personal dedication, rather than the mandatory requirement of an external organization.† To me this statement means that as a professional counselor I will further my education  and knowledge of skills because I want to and not because an agency I work at mandates it. I do not believe that when one finishes school, they have completed learning, especially if they work in behavioral health. There are always new diagnoses, methods, and forms of treatment coming out that I want to be learn to better meet the needs of my future clients. In a mail-in survey study of 1000 licensed professional counselors conducted in 2009 pertaining to counseling grief stricken clients, Granello, Ober, & Wheaton (2012) found that the majority of the participants stated they were unprepared when it came to specific skills and lacked knowledge to address those with grief. â€Å"Counselors who received training rated themselves as more competent than those who did not, with more training related to higher levels of self-perceived competence† (Granello et al., 2012, p. 158). Another study conducted by Jameson, Poulton, and Stadter (2007), involved 38 therapists and evaluated the effect of a two-year continuing education program on their knowledge, skills, and application. â€Å"The majority (74%) felt the training helped them think clearly and specifically, both about assessment issues and specific interventions† (Jemeson et al., 2007, p. 113). It is clear when reading these findings that further training can only help a professional to work with more specific needs of their clientele. Although all agencies have mandatory trainings, I have observed that there are hundreds of additional trainings offered yearly for any counselors who want to attend voluntarily. I plan to be a counselor who takes the opportunities offered to further educate myself, in order to improve myself and to provide my clients with a better and more knowledgeable version of me. Conclusion In summation, I have examined a potentially personally challenging section of the ACA Code of Ethics, risk management strategies that I plan to utilize, and a section of the ACA Code of Ethics that aligns with my personal beliefs. Examining my personal experiences and traits that may conflict with the ACA Code of Ethics, I am better preparing myself to prevent any effects they may have had toward my future clients. It is important to me that I continue to learn and apply the knowledge I gain in graduate school and additional educational settings to improve myself as a person and as a professional counselor. History of Psychology Essay Modern psychology is a science that it continually being researched and added to. Psychology predates the 19th century and includes roots into early philosophy. Looking closely one can identify philosophers that related to the beginnings of psychology, identify major philosophers that historically relate to the beginnings psychology as a formal discipline and how the development of the science of psychology changed during the 19th century. A philosopher that could easily be called the father of modern psychology would be: Rene Descartes. Descartes was a philosopher in the 17th century that stepped outside the limits and wanted to know more. Descartes came up with 4 basic rules to arrive at the truth of whatever he was researching. These rules were basically to think clearly, logically and without bias (Goodwin, 2008). Descartes had several derived ideas and was considered a nativist and a rationalist. These ideas that he created foreshadowed one of psychology’s major topics of nature versed nurture. Descartes also fought that there was a difference between a person’s mind and body; and this difference separated us from animals. Before Descartes died he published his last of many books called: The Passions of the Soul. This book established him as a psychologist and a physiologist (Goodwin, 2008). This book emphasized on human emotions and continued on with a discussion about mind and body distinctions. He Clarified the body is a machine and responds to curtain motions, such as fear, that trigger reflexes. Descartes also determined that the pineal glad in the brain was the gland that sent messages from the mind/spirit to the body. Descartes died at just before his 54th birthday in 1650. There were several philosophers that historically relate to the beginnings psychology as a formal discipline. John Locke, George Berkeley, David Hume and John Stuart Mill are just a few to mention. Each philosopher contributed an aspect to psychology that led to its formal discipline. John Locke refused to believe in an innate mind and believed that mind is created by experiences and was not pre programmed. He stated believed that the ideas that come from our minds are due to ones sensations and reflections from experiences (Goodwin, 2008). George Berkeley added an analysis of visual perception. Discuss Effect Of Good Nutrition On Wound Management Nursing Essay This essay will discuss nutrition and the effects it has on wound management, and what impact communication skills have on patients health. I will include some information about my experience of wound management, while working along side my mentor in a rehabilitation unit, for a patient who was admitted with multiple scleroses (M.S), the patient is wheel chair bound and had a pressure sore which developed into a sacral sinus wound (grade 4). In accordance with the Nursing and Midwifery Council (2008), Guidelines on Confidentiality, I will refer to the patient as Ben, who is 72 years old. Bens wound had caused extensive destruction of his tissues and damaged to his muscle and supporting structures. A swab was taken and reports confirmed that his wound had been infected with (Methicillin-resistant Staphylococcus aureus) MRSA. Flanagan(2000) states that age reduced mobility, malnutrition, incontinence, skin integrity, friction, moisture, and pain. It can lead to skin breakdown and wound developing. Bens observation were taken regularly, he had a supra pubic catheter, which was monitored on regular basis to minimize the risk of infection, Bens bowel motion was also assessed; it was made sure that the wound would not be contaminated with overflow of bowel. His over all conditions were monitored every day but his dressing were not changed every day. The nurse, my mentor, was using a local wound assessment chart for managing his wound. The chart was used for the assessment of his wound and every time it was updated after changing the dressing. All changes and appearance of the wound were noted down in the chart. Progress in daily assessment and plan of care were noted down in the chart to carry out regular reassessment of his wound. According to Eunis and Menesis (2000), the excess of exudates within the wound can also inhibit healing, control of exudates is therefore essential. This is usually achieved by selecting a dressing of the appropriate absorbency. The nurse selected an Aqua Cell Silver dressing to use for Bens wound, as his wound was infected with MRSA. (Methicillin Resistant Staphylococcus Aureus). British National Formulary (2006) says that aqua cell silver is an appropriate dressing for infected wounds. Gunnewicht and Bun (2004) described that if the wound is clean, healthy, granulating and happy; it does not require cleaning because the wound exudate itself has beneficial bactericidal properties, which may be inappropriateely removed. The general strategy of my mentor, in the cleansing of Bens wound was based on providing minimal necessary intervention. She was using normal saline to clean the wound. Griffiths et al (2001) stated that the solution should be of a non- irritant and free of bacteria. Normal saline is the most commonly used wound cleaner, it is best to use the solution at body temperature. We identified and addressed the nutritional needs of Ben. This was of a puree diet and the supervision of his meal times, to aid optimum recovery. A dietition was envolved in Bens nutritional needs. It is obvious that nutrition plays a crucial role in wound healing. All patients with wounds should have appropriate nutritional assessment. If a patients nutritional status is compromised, and they are unlikely to meet their requirements, recovery will be delayed. Boon (1998) said that a good nutritional assessment involves the multidisciplinary approach including medical, nursing and dietretic staff. Ben was kept on a food chart which was updated everyday after each meal. The correct quantities of Bens intake were entered on the chart. If we examine the importance of nutrients, we would know that they have key roles in the healing process, such as, protein depletion can affect the rate of wound healing because it is required for the granulation of tissues, carbohydrates are also a part of healing process, cellular activity is fuelled by adenosine triphosphate (ATP), which is derived from glucose, providing the energy for the inflammatory response to occur. If carbohydrates are not sufficient, the body breaks down proteins to provide glucose for cellular activity. Therefore, carbohydrates are required as well as proteins. Ben was given fruit yougurt or pudding after every meal, mash potato and meat were the regular part of his diet along with some mashed vegitables. Fats and vitamins have a key role in cell membrane structure and function, certain fatty acids are essential as they cannot be synthesised in sufficient amounts, so must be provided by diet. Williams and leaper (2000) states that B complex vitamins are co-factors or co-enzymes in a number of matabolic functions involved in wound healing, particularly in the energy release from carbohydrates. The doctor had prescribed 30mg of zinc per day for six weeks, along with multivitaminal tablets. According to Grey and Cooper (2001) vitamin C has an important role in collagen synthesis in the formation of bonds between strands of collagen fibre, while vitamin K is involved in the formation of thrombin. Deficiency of vitamin K (in the presence of wound) can lead to haematoma. Vitamin A supports the proliferation of epithelial cells. Minerals like zinc, iron and copper play a vital role in wound healing. Zinc is required for protein synthesis and also has an inhibitory effect on bacterial growth; it is envolved in the immune response. Long term of zinc supplimentation must be accompained by copper supplimantation to prevent zinc induced copper deficiency. The deficiency of copper and iron delay wound healing.Perkins (2000), defined that the ideal way to meet requirements of nutritional needs, is by consuming adequate intake of normal food. My mentor and I were spending time with Ben, explaining and educating him about the process of his wound healing and the importance of medication, nursing interventions and nutrition. I felt that Ben needed support and care for his treatment. According to Quality and Safety in Health Care (2008) Communication looks easy when it is done well, it requires engagement, empathy, and ability to listen and respond, and it requires time. There are two types of activities which are being conducting, one is school based activities and another one is class based activities especially for language subjects. One of the best example is Majlis Cemerlang UPSR (Ujian Peperiksaan Sekolah Rendah ). This program will be held on January every year for students who achieved more than 3As in UPSR. This activity is headed by the headmaster of the school and teachers will be the coordinators of this program. The main aim of this program is to reward students who contribute excellence results for the school and this will be a source of motivation for other students to achieve a better result for upcoming years by being a role-model. Other than that, the headmaster also will give a talk on KSSR and also about the band systems for parents who enrol their children in year 1. Activities like 100 meter race, 200 meter race, shot put and others will take place on sport day. On the actual day, president of PIBG (Persatuan Ibu Bapa Guru) and parents are invite to give supports for students to do better performances. Medals will be given for students who win for each and every activity. The implications from this sport day are to produce students with high discipline level and balanced according to JERIS (physically, emotionally, spirituality, intellectually and socially). This program is really useful for students because they can learn more about the purposes of joining such program like this and may share their experiences with other students. So, other students will have the interest to join the program on other time. NPE will be used as a base for this program in order producing generation of students with high vision by creating awareness and understanding of the organizational principles which allow students to learn through this program. Lastly, school based activity which conducts by the school is Science Fair. The activity really interesting when found out about the students achievement that had made last year. October will be the month of Science where the science teachers struggle hard to prepare experiments for each class according to levels such as lower and upper primary. In addition, the purpose of conducting school level Science fair is to give exposure on Science world and make up students mind that Science is really fun to learn by doing the experiments and it is not difficult if learn in a proper way. Teachers in this school will guide students to conduct the experiments and rewards will be given for the most interesting and successful experiments. Parents are invited because to give encouragement to students and to give clear picture about students who are being as a Science class minded. Students can create own initiative to modify their language proficiency that will be useful when they are complete their primary school level.
